Employee Benefits Information


2024 - 2025 OPEN ENROLLMENT

Effective Monday, April 22, 2024 - Friday, May 3, 2024 we will be having our annual open enrollment.  Current elections for health, dental life, voluntary accident and voluntary critical illness will roll over to the next plan year.  HSA and Flex elections will NOT.  Employee's have to re-elect HSA and Flex elections for 24-25.

HEALTH PLAN INFORMATION 
 

Health insurance is offered through United Health Care to all employees eligible to receive benefits.  Single, employee + one and family coverage is offered.  Please refer to your working agreement to see what plan you qualify for and what the monthly premiums will be.  The plan summaries are found by clicking on detailed plan information, page 2 under the health plan documents.

DENTAL PLAN INFORMATION 
 
Dental  insurance is available through Delta Dental of MN to all employees eligible to receive benefits. Single and family coverage is offered. The Delta Dental Program pays 100% for diagnostic and preventive services. Other dental services are covered at 80% or 50% after an annual deductible of $25 per covered person. The program pays up to a maximum of $1000 for each covered person per calendar year. Orthodontic Services are covered at 50% up to a separate lifetime maximum of $1000 per covered person. Please see the plan summary or health care contract for complete detail of coverage. 
 
EMPLOYEE ASSISTANCE PROGRAM

TELUS Health One (formerly LifeWorks) is the contact for our free Employee Assistance Program. Phone consultation~24 hours per day 365 days per year and face to face counseling for you and your family members is available. For access to web-based work and life resources call 866-451-5465 or visit www.niseap.com.  Log in enter Farmington ISD 192,  username enter NISEAP and password enter EAP. They are available to help with marital, relationship and parenting concerns, alcohol and drug abuse issues, depression, stress and anxiety, legal and financial problems and much more.  Also included is the LIFT program.  LIFT is virtual wellness and fitness sessions.


FLEXIBLE SPENDING PLAN INFORMATION
 
 
The Flexible Spending Plan permits employees to reclassify a portion of their paycheck into a pre-tax position. The employee can choose to contribute to a Medical Flexible Spending Account(Medical FSA) and/or a Dependent Care Flexible Spending Account (Dependent Care FSA). The money the employee elects to contribute is deducted from their paycheck on a regular schedule. 
 
LIFE INSURANCE INFORMATION 
 
Term life insurance is available through Madison National Life for employees eligible to receive benefits. The amount of coverage is outlined in your master agreement. A voluntary supplemental life insurance policy is available to employees who meet eligibility requirements, at the employees expense. 
 
LONG-TERM DISABILITY (LTD) INSURANCE INFORMATION  
 
Long-term disability insurance is provided through Madison National Life for employees eligible to receive benefits. Through LTD insurance, compensation may be available if an employee is unable to work because of sickness or bodily injury. If qualified, benefit payments of up to 66-2/3% of the employee's monthly income can begin after a 60-day period of total disability. 

VOLUNTARY VISION INSURANCE

Voluntary vision insurance is available through VSP for all employees who are eligible to receive benefits. Single, Employee +1 and Family coverage is offered at the employee's expense.
