HEALTH PLAN INFORMATION

DENTAL PLAN INFORMATION 
 
Dental  insurance is available through Delta Dental of MN to allemployees eligible to receive benefits. Single and family coverage isoffered. The Delta Dental Program pays 100% for diagnostic andpreventive services. Other dental services are covered at 80% or 50%after an annual deductible of $25 per covered person. The program paysup to a maximum of $1000 for each covered person per calendar year.Orthodonic Services are covered at 50% up to a seperate lifetime maximumof $1000 per covered person. Please see the plan summary or health carecontract for complete detail of coverage. 
 
EMPLOYEE ASSISTANCE PROGRAM

Bensinger Dupont & Associates is the contact for our free Employee Assistance Program. Phone consultation~24 hours per day 365 days per year and Face to face counseling for you and your family members is available. For access to web-based Work and Life resources call 866-451-5465 or visit www.niseap.com and enter NISenhanced as the password.  They are available to help with marital, relationship and parenting concerns, alcohol and drug abuse issues, depression, stress and anxiety, legal and financial problems and much more.


FLEXIBLE SPENDING PLAN INFORMATION
 
 
The Flexible Spending Plan permits employees to reclassify a portion oftheir paycheck into a pre-tax position. The employee can choose tocontribute to a Medical Flexible Spending Account(Medical FSA) and/or aDependent Care Flexible Spending Account(Dependent Care FSA). The moneythe employee elects to contribute is deducted from their paycheck on aregular schedule. 
 
LIFE INSURANCE INFORMATION 
 
Term life insurance is available through Madison National Life foremployees eligible to receive benefits. The amount of coverage isoutlined in your master agreement. A voluntary supplemental lifeinsurance policy is available to employees who meet eligibilityrequirements, at the employees expense. 
 
LONG-TERM DISABILITY (LTD) INSURANCE INFORMATION  
 
Long-term disability insurance is provided through Madison National Lifefor employees eligible to receive benefits. Through LTD insurance,compensation may be available if an employee is unable to work becauseof sickness or bodily injury. If qualified, benefit payments of up to66-2/3% of the employee's monthly income can begin after a 60-day periodof total disability.
