Early Childhood Screening

Update 7/14/2020: The Early Childhood Screening program in Farmington is following the Minnesota Department of Education guidelines. Our first concern is for the welfare of the families in the Farmington School District. Due to the need for social distancing, face-to-face screening cannot take place at this time. In the meantime families can start the process by filling out the required forms now. Completing and submitting these forms ahead of time will result in a shorter screening time when in-person screening resumes. To start with the Early Childhood Screening forms, please enter your information Here
A screening team member will contact you after receiving your information for more forms to fill out.

Those families who had an appointment that was cancelled, have a child starting Kindergarten in the fall, or require Early Childhood Screening for an Early Learning Scholarship will be given priority appointments when screening resumes

This situation is evolving, our office will continue to communicate with you as more is known. If you have a child entering Kindergarten this Fall, rest assured that this delay will not affect their Kindergarten entrance eligibility.

If you have any questions, the best way to contact us is by email.
