Early Childhood Screening

Update 11/19/2020: The Governor has announced a new executive order last night that will affect our community for December. The Farmington School District is pausing all in-person learning from Dec. 1 to Dec 22nd. Our screening sessions for December 7, and December 16 are canceled. Our goal will be to provide in-person Early Childhood Screening as soon as it is determined safe to do so. We will continue to communicate with you as we know more.

In the meantime, we are asking families to start the process by filling out the required forms first. Completing and submitting these forms ahead of time will result in a shorter face to face screening time when we return. To start with the Early Childhood Screening forms, please enter your information here.

If you have any questions, the best way to contact us is by email.
