Homeschool Information

In accordance with state law, Farmington Area Public Schools are required to ensure that all school-age children living within the district’s boundaries are being educated. The district’s primary role is to be certain that all Home Schools comply with state law and to provide the necessary information and forms to each Home School for compliance. Current Home School families in ISD 192 will receive an e-mail in August from the district that will direct them to this website for sample forms requesting the information to be submitted for the 2023-24 school year.

Home School Registration Information for 2023-24
Forms listed below as "Mandatory" must be submitted before the specified deadlines. If complete information (immunizations, parent degrees/certification, school calendar days and norm referenced testing information if required) is not received, the district must report the Home School as “Not Compliant” in our report to the State of Minnesota in November.

Homeschool Full Report - Due Oct. 1

School Calendar - Due Oct. 1
You must submit your instructional calendar for the year by Oct. 1. The District 192 Calendar is available for your reference.

Student Immunizations - Due Oct. 1
You must submit a Student Immunization record by Oct. 1. You can download the form and learn more about required immunizations at the District 192 Health Services page.
