Immunization Requirements

Minnesota Immunization law requires that children enrolled in school to be immunized against certain disease or file a legal medical or conscientious exemption with the school.

An updated immunization record with booster shots received or exemption is required when
a child enters kindergarten and 7th grade. Please see the document below entitled "Are Your Kids Ready" to see which immunizations are required at the various ages.

Forms:
Parents may file a medical or conscientious objection on the Student Immunization Form located below. This form must be signed by a parent or guardian in front of a notary public.

The immunizations or exemption is due to be on file by the first day of school.

Child Health Clinic

The Dakota County Child Health Clinic provides regular health exams and immunizations to well infants, children and teens in Dakota County. Well Child Exams and immunizations are available to those that do not have health insurance or whose health insurance does not cover the cost of vaccines. Please call (952) 891-7526 with your questions or visit www.DakotaCounty.us (search Vaccines).
